Item Building Effective Drinking Water Management Policies In Rural Africa: Lessons From Northern Uganda(CIGI-Africa Initiative, 2012) Opio, ChristopherThe importance of providing clean, safe drinking water and sanitation to rural inhabitants of developing countries is widely recognized. The United Nations (UN) General Assembly, for instance, declared 2008 the International Year of Sanitation, and the World Bank has been increasing financial assistance to developing countries in support of water supply and sanitation improvements (Cho, Ogwang and Opio, 2010). Despite the Millennium Development Goal (MDG) to reduce, by half, the number of people without sustainable access to clean and safe drinking water and basic sanitation by 2015 (Cho, Ogwang and Opio, 2010; Opio, 2010), most countries in Sub-Saharan Africa are not on track to meet the widely adopted deadline (Harvey, 2007; United Nations Children’s Fund [UNICEF], 2012; Abenaitwe, 2012).Item Elemental composition and potential health impacts of phaseolus vulgaris L. ash and its filtrate used for cooking in northern Uganda(African Journal of Food, Agriculture, Nutrition and Development, 2016-12-06) Opio, Christopher; Bergeso, T. L.; Arocena, J. M.Ash from burnt crop residue of common bean (Phaseolus vulgaris L.) is typically used to generate filtrate in rural Northern Uganda. The filtrate is added to hard-to-cook foods, like dried legumes, to decrease cooking time and improve flavor. However, the elemental composition of ash filtrate and health implications of its use is poorly understood. This study aimed to determine the elemental composition of Phaseolus vulgaris L. ash and its filtrate, to identify variation among study sites, and to assess the potential health impact of ash filtrate consumption in Northern Uganda. Dried ash and ash filtrate samples of P. vulgaris from Dog Abam, Telela, Arok, and Tit villages in Northern Uganda were analyzed for chemical composition. Ash filtrate samples were procured from ash according to local methods. Nutritional impact was assessed by comparing recommended daily intake (RDI) guidelines for Canada and Uganda. Potassium (K), sodium (Na), magnesium (Mg), manganese (Mn), and iron (Fe) concentration in dry crop ash samples varied significantly among study sites. Ash filtrate contained lower concentrations of all elements, suggesting considerable losses through filtration; but showed an alkaline pH (10.1 to 10.8). Elemental concentration present in probable daily intake of ash filtrate (approximately 15 milliliters/person) was within acceptable RDI ranges for elements of known dietary importance. The alkaline pH levels of the ash filtrate may have potential negative effect on diet by decreasing bioavailability of specific minerals (for example, Fe and Zn) and/or having destructive effects on various nutrients (for example thiamine). Further research should be conducted in Northern Uganda and other areas where ash filtrate is in use to determine the specific health effects of this cultural practice. Such studies could include, but not limited to, biological analysis, detailed nutritional studies, and/or long-term monitoring of filtrate consumers. The information gathered from such studies could be critical in formulating appropriate policies regarding the use of ash filtrate.Item Ethical and Scientific Considerations on the Establishment of a Controlled Human Infection Model for Schistosomiasis in Uganda: report of a stakeholders’ meeting held in Entebbe, Uganda.(AAS open research, 2018) Elliott, Alison M.; Wajja, Anne; Opio, Christopher; Angumya, Francis; Adriko, Moses; Egesa, Moses; Mfutso-Bengo, Joseph; Kapulu, Melissa; Lutalo, Tom; Nazziwa, Winfred Badanga; Muwumuza, Asuman; Kaleebu, Pontiano; Kabatereine, Narcis; Tukahebwa, EdridahControlled human infection (CHI) models are gaining recognition as an approach to accelerating vaccine development, for use in both non-endemic and endemic populations: they can facilitate identification of the most promising candidate vaccines for further trials and advance understanding of protective immunity. Helminths present a continuing health burden in sub-Saharan Africa. Vaccine development for these complex organisms is particularly challenging, partly because protective responses are akin to mechanisms of allergy. A CHI model for Schistosoma mansoni (CHI-S) has been developed at Leiden University Medical Centre, the Netherlands. However, responses to schistosome infections, and candidate vaccines, are likely to be different among people from endemic settings compared to schistosome-naïve Dutch volunteers. Furthermore, among volunteers from endemic regions who have acquired immune responses through prior exposure, schistosome challenge can be used to define responses associated with clinical protection, and thus to guide vaccine development. To explore the possibility of establishing the CHI-S in Uganda, a Stakeholders’ Meeting was held in Entebbe in 2017. Regulators, community members, researchers and policy-makers discussed implementation challenges and recommended preparatory steps: risk assessment; development of infrastructure and technical capacity to produce the infectious challenge material in Uganda; community engagement from Parliamentary to grass-roots level; pilot studies to establish approaches to assuring fully informed consent and true voluntariness, and strategies for selection of volunteers who can avoid natural infection during the 12-week CHI-S; the building of regulatory capacity; and the development of study protocols and a product dossier in close consultation with ethical and regulatory partners. It was recommended that, on completion, the protocol and product dossier be reviewed for approval in a joint meeting combining ethical, regulatory and environment management authorities. Most importantly, representatives of schistosomiasis-affected communities emphasised the urgent need for an effective vaccine and urged the research community not to delay in the development process.
